By attending the Smile PARADISE Dance Festival 2023 on 19-23 October at Studio Dance 361 8600 Boulevard Parkway - ANJOU, I, the undersigned, acknowledge and agree to the following:
Assumption of Risk: I understand that attending and participating in the Smile PARADISE Dance Festival 2023 carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. I acknowledge and willingly assume all risks associated with participation.
Waiver and Release of Liability: I hereby release, waive, discharge, and hold harmless Smile PARADISE Organization, its officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action related to any loss, damage, injury, or death that may be sustained by me or to any property belonging to me while participating in the event.
Personal Responsibility: I acknowledge that I am an adult and that I am responsible for my own well-being and safety during the event. I will ensure that I am in a state of health appropriate for my participation and will judge for myself my capacity to participate.
Medical Treatment: I give my consent to [Your Organization or Name] and its representatives to seek medical treatment on my behalf in the event of an emergency.
Photo and Video Release: I understand that during the event, I may be photographed or recorded on video. I hereby grant Smile PARADISE Organization the right to use such photos or videos for promotional and archival purposes.
Acknowledgment: I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights by signing it. I acknowledge that I am signing this agreement freely and voluntarily.
